Accountability Coaching Support Plan

​

Structured and comprehensive monitoring for individuals wanting consistent oversight and support.  This service supports individuals working on their personalized substance use goals, legal requirements, and/or family and professional oversight needs.

Includes:

•  4 DNA-verified urine drug and alcohol tests each month

•  Initial set up and onboarding support

•  Secure results shared with approved individuals

•  Phone and text support

•  Accountability check-ins with experienced substance use counselors

•  Education on results and individual substance use goals

•  Customized result summaries for individuals, families, legal reps, and/or clinical teams

•  Coordination with treatment providers and/or legal representatives

• Additional test(s)

• Letter for court requirements and/or employers

Confidentiality is our priority.  All results, communication, and support are handled with the upmost discretion and care.  No information is shared without written authorization.

Rise Up To Change provides comprehensive at-home, DNA-verified testing (100+ substances), expert oversight, education, and personalized support. Comparable weekly testing at an independent lab typically costs significantly more and lacks the same privacy, convenience, and ongoing guidance. We are a private-pay service but provide Superbills for clients who wish to independently submit to their insurance company for possible reimbursement.
